Please complete the following Account Closure/Sale form which is used for the reasons listed:
  • had a change in ownership,
  • sold,
  • closed,
  • no longer operating in Saskatchewan,
  • cancelling Optional Personal Coverage (OPC) or,
  • business amalgamation/restructure has occurred.

This form can be submitted online in two ways:
  1. Online Services Account - Employers who have set up an online account can submit the ACF through this account.
  2. As a guest - This service allows you to submit this form without any special registration
